1. Create your account
2. Complete the online application form for students/ volunteers
3. Upload photos of your 100points of ID ensuring that one of the photos is of yourself, holding up a photo ID document next to your face to match you to your ID documents.
   (eg. Drivers Licence, Passport, Photo ID card). You can do this by selecting the "Do you want to take your photo now?".
4. Enter your payment details and submit the application to us for processing
5. Bring photo ID to Coach Training Day 1.

NOTE: Please ensure all information provided in your application is exactly as it appears on your identity documents.

A Working With Children Check (WWCC) is a requirement for people who work or volunteer in child-related work. It involves a national criminal history check and a review of findings of workplace misconduct; and the result is either a clearance to work with children for five years, or a bar against working with children.

The process for applying for a WWCC is explained in this short video and is as follows:

1. Fill in an online application form as a volunteer and record the Application Number you receive.
2. Visit Services NSW and present your Application number and proof of identity to complete your application.
3. You will receive your results by email (which may take a few weeks).
4. Email the WWC number to your Program Manager for verification.

The following questions are required to meet the national reporting requirements of a Registered Training Organisation (RTO). Please answer them to the best of your ability.

Acceptance into the Max Potential program as a participant is based on:

  • Signed Consent with Parent/Guardian Consent (Parent/Guardian Consent is only required if you are under 18).
• Applicant’s commitment to the Max Potential program (as below)

• Prioritise Max Potential in my schedule and attend all group activities and coaching sessions.
• Commit to personal growth and active development of my personal leadership skills.
• Fulfil the agreement I make with my coach and commit to developing an open, honest and transparent coaching relationship.
• Work to understand the MAXIMISERS principles and apply them to my life
  • Be clear about what success looks like.
  • Try new ways of thinking and doing things to achieve my goals.
  • Create space in my day to focus on new habit formation.
• Complete all requirements of the program within the required timeframe, including a
  • Personal Expression to be presented to the group.
  • Community Service Project to help others and reinforce my personal learning.
  • Certificate 1 in Active Volunteering (not compulsory - only if I choose to enrol).
  • Showcase display to celebrate my success.
  • All MP Hub online documentation including workbook.
• 'Push through' any challenges that arise to make the most of my Max Potential experience.
  I understand that failure to complete the program for non-extenuating circumstances will require me to repay the withdrawal cost of this scholarship ($600+GST).

Acceptance into the Max Potential program as a Community Coach is based on:

• Completion of this online application form.
• Providing a valid Working With Children Check number before Coach Training Day 2.
• 100-point identity check.
• A cleared Police Check before Coach Training Day 2.
• Attendance at coach training (two full days) and demonstrated coaching capability
  Important note: The Max Potential Program reserves the right to only appoint individuals as coaches who have completed all activities and demonstrate an appropriate attitude and coaching competence after coach training.

• Complete all requirements of the program within the required timeframe, including
  • All MP Hub online documentation including approval of Young Adult’s workbook as required.
  • Coach Young Adult(s) to achieve their goals and successfully meet all program outcomes on time:
    • Personal Expression to be presented to the group.
    • Community Service Project to help others and reinforce their personal learning.
    • Certificate 1 in Active Volunteering (not compulsory - only if they choose to enrol).
    • Showcase display to celebrate their success.
• 'Push through' any challenges that arise to make the most of my Max Potential experience.
  If I have received a Coach Scholarship, I understand that failure to complete the program for non-extenuating circumstances will require me to repay the full cost of this scholarship ().
